Community Outreach Worker

This temporary position is grant-funded and ranges from 12 to 18 months and will perform community outreach and health education activities.

PURPOSE STATEMENT: This temporary position is grant-funded and ranges from 12 to 18 months. The Community Outreach Worker will perform community outreach and health education activities to support the My First 1,000 Days: A Dairy Initiative project.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below represent knowledge, skill and ability. ESSENTIAL DUTIES AND RESPONSIBILITIES: This list of duties and responsibilities must be listed by importance or priority. Other duties may be assigned.

COMMUNITY OUTREACH WORKER KNOWLEDGE, SKILLS AND RESPONSIBILITIES:

  • Assist in implementing the project's task through collaboration with program staff:
    • Participating in the project participant recruitment and enrollment activities.
    • Communicate any issues with the supervisor promptly.
    • Assist with the documentation of participant enrollment into the project following a standard protocol this will include using Microsoft Excel to track participant information.
  • Conduct one-on-one assessments, interviews and screenings with participants following a standardized protocol.
  • Assists in organizing and attending community events and presentations.
  • Serves as a support for participants by facilitating voucher distribution appointments that are mutually acceptable.
  • Communicate changes to working hours with the supervisor weekly.
  • Build rapport and connection with participants empathetically, willing to listen and validate their feelings.
  • Ability to communicate effectively by telephone, in person and digitally. This includes texting, phone calls, emails, site visits and video conferencing, as necessary.
  • Evidence of strong people skills and teamwork.
  • Basic skills in using computer software and apps.
  • Knowledge of the region and population being served:
    • These activities take place in both Merced and Stanislaus County.
    • Knowledge of the local bilingual community.
    • Ability to get to community locations promptly.
  • Knowledge of ethical standards of practice, discretion and confidentiality.
  • Coordinate appropriate referrals from providers and community-based organizations; maintain records for the referrals and help to close the communication loop.
  • Demonstrate strong knowledge of currently available local resources and the ability to seek out additional resources as needed; remain current on changes in resource availability and eligibility criteria.
  • Foster positive working relationships between local resources, retailers, and partners; provide regular updates to supervisor and team.
  • Promote increased knowledge, awareness, increased consumption and sales of dairy products.
  • Provide technical assistance to retailers and ensure dairy products are stocked for participant purchase.
  • Work with store owners and/or managers to successfully track redemption of vouchers.
  • Perform additional duties as assigned within the Community Outreach Worker roles.  

SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities.

EDUCATION and EXPERIENCE: Have graduated from an accredited high school or possess a General Educational Development certificate. One to two years of experience working in the public health or health science field, focusing on performing community service work, providing information and referral services, or promoting community activities. Must be comfortable communicating through writing, presentations, and exhibiting at conferences. Must be computer literate and be able to use standard business software and applications.

LANGUAGE SKILLS: Bilingual English/Spanish (fluent) required. Ability to respond, verbally and in writing, to common inquiries from project participants. Ability to effectively present information, verbally or in writing, to partners, providers, and professional groups. Ability to adapt nutrition information in various formats appropriate for the targeted audience.

MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

PHYSICAL DEMANDS: The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. Upon the employee's request, reasonable accommodation may be made to enable individuals with disabilities to perform essential functions.

While performing the duties of this job, the employee is regularly required to talk or listen. The employee must frequently sit and use their hands to finger, handle or feel. The employee is occasionally required to stand, stoop, kneel or crouch. The employee must frequently lift and move up to 10 pounds and occasionally lift and move up to 25 pounds. Specific vision abilities required include close vision, color vision and the ability to adjust focus.

WORK ENVIRONMENT: Upon the employee's request, reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. The noise level in the work environment is usually quiet.

OTHER QUALIFICATIONS: Travel is required. Must be able to travel around California 30-40% of the time (specifically Merced and Stanislaus County). Ability to work flexible hours that may include weekends and evenings. It is necessary to have a reliable vehicle (for which travel is reimbursed according to company policy) and possess and maintain a valid California driver's license and personal automobile insurance. Must be able to establish, foster and maintain positive and harmonious working relationships with those contacted in performing duties and to be able to work in a team setting within the Dairy Council of California.
